Claims
At Protective Life Insurance Company, taking care of beneficiaries is one of the most important things we do.
If you've lost someone you love, the last thing you want to think about is your financial affairs. To that end, we do our best to make sure beneficiaries receive all the benefits to which they are entitled as quickly and smoothly as possible.
To file a claim, you'll need to submit three important things:
- A completed Statement of Claim, a form that includes such things as your full name and address (or those of the beneficiary or beneficiaries). Call 1-800-424-1592 to obtain the right form.
- A certified death certificate showing the date, cause and manner of death.
- The original insurance policy if available or a lost policy receipt if not available.
